Job summary

Rainbow Community Care is seeking dedicated home hospice CNAs to provide high-quality home care in the Northern Dodge County area. The role includes day shifts with on-call, weekend, and holiday rotations, driving to patients’ homes, maintaining records, and supporting patients and families in illness and loss.

Qualifications

• Certification as a Nursing Assistant and valid registration with the Wisconsin CNA Registry required.
• Minimum of 6 months CNA experience preferred.
• Hospice experience is preferred.
• Flexible to work weekend and holiday rotation.
• Ability to drive to patients' homes and meetings as necessary.

Responsibilities

• Assist with ambulation and transfers.
• Maintain a clean, safe and comfortable environment.
• Prepare meals and assist patients with eating.
• Provide emotional and psychological support to the patient/family.
• Report status/observations per RCC guidelines.
• Maintain up-to-date patient records and document care and patient condition.
• Assist with activities of daily living and light housekeeping.
• Perform other duties as assigned.
