Job summary

BrightStar Care Southwest Austin, TX is hiring a Registered Nurse for home health visits in Austin and surrounding areas. This full-time and PRN role offers competitive per-visit compensation for SOCs ($90) and Treatments ($50) with weekly pay and mileage reimbursement.

You will conduct assessments, develop care plans, administer medications, document in EHR, and collaborate with a care team. Texas RN license, BLS/CPR, valid driver's license, and ability to work independently are required; OASIS

Qualifications

• Current Texas RN license (compact accepted).
• Current BLS/CPR certification.
• Valid driver's license and reliable transportation.
• Strong clinical skills and knowledge of home health care practices.
• Ability to work independently and manage time efficiently in the field.

Responsibilities

• Assess patients' medical conditions and needs.
• Create and implement individualized care plans with the team.
• Administer medications, treatments, and procedures as prescribed.
• Monitor responses and adjust care plans as needed.
• Complete OASIS, ICD-10 coding, and timely notes in EHR.
• Maintain accurate documentation and communicate with families and team.

Treatments & discharges $50/per visit- Ongoing skilled visits

JOB DUTIES

• Conduct comprehensive assessments of patients' medical conditions and needs
• Develop and implement individualized care plans in collaboration with the team
• Administer medications, treatments, and procedures as prescribed by physicians
• Monitor patient responses to treatment and adjust care plans as needed
• Complete accurate OASIS documentation, ICD-10 coding, and timely visit notes
• Maintain accurate documentation in the electronic health record (EHR) system
• Communicate effectively with patients, families, and the broader care team

Required qualifications

• Current, valid RN license in the state of Texas (compact license accepted)
• Current BLS / CPR certification
• Valid driver's license and reliable transportation
• Strong clinical skills and knowledge of home health care practices
• Ability to work independently and manage time efficiently in the field

Preferred qualifications

• 1+ year of home health or clinical nursing experience
• Familiarity with OASIS documentation and Medicare home health regulations
