American Traveler, LLC. is seeking a Home Health RN Float with an active MA RN license and at least 1 year of certified home health and cas e management experience.

The role covers Lynn, Nahant and Saugus in the North Region with a 13-week assignment, 08:00-16:30 shifts, and weekend rotation every six weeks. Responsibilities include patient assessment, care plan implementation, and family education in home settings, plus admissions and ongoing case management per policy.

Qualifications

• Active MA RN license; no pending or conditional licenses.
• Minimum 1 year of experience in a certified home health agency.
• Minimum 1 year of case management in a certified home health agency.
• AHA-issued certifications required.
• Flu vaccination required; no religious or medical exemptions.
• License verification via NURSYS and BON.

Responsabilités

• Cover Lynn, Nahant, and Saugus in the North Region.
• 13-week assignment; 08:00-16:30 shift.
• Weekend rotation every 6 weeks.
• Travel to patient homes as assigned.
• Use Epic for charting and documentation.
• Assess patient conditions and implement care plans; educate patients and families.
• Admission processes and case management per policy.
• Timely electronic documentation and interdisciplinary communication.
• Care aligned with community health standards and organizational philosophy.
• Local candidates within 60 miles of Massachusetts General Hospital in Boston considered at a reduced rate.
• Timekeeping via UKG/Kronos.
