Job summary

CenterWell, a Humana company, is seeking a Home Health Speech Language Pathologist to evaluate and treat patients in home or facility settings. You will develop plans of care and coordinate with the care team to improve communication, swallowing, and overall quality of life for patients.

The role requires ASHA CCC or equivalent, 6+ months experience, a current license, CPR, and reliable transportation. Schedule is around 20 hours weekly with pay-per-visit compensation and benefits as described

Qualifications

  • Certification of Clinical Competence in Speech Language Pathology or Audiology per ASHA.
  • Minimum six months experience as a Speech-Language Pathologist.
  • Current and unrestricted license.
  • Current CPR certification.
  • Valid driver’s license, auto insurance, and reliable transportation required.

Responsibilities

  • Evaluate, direct and provide speech-language pathology services in patients' home or facility.
  • Develop and review the Plan of Treatment and Plan of Care.
  • Assess and treat disorders of speech, voice, language, hearing and swallowing.
  • Administer and interpret diagnostic tests and therapeutic applications.
  • Observe and report changes in patient condition to supervisor or physician.
  • Provide instruction to patients in alternative communication systems when appropriate.
  • Counsel patients, families and healthcare staff.
  • Maintain and submit documentation as required by the Company.
  • Participate in care coordination and discharge planning.
  • Maintain confidentiality and handle information appropriately.

By delivering personalized care that focuses on rehabilitation and functional improvement, you’ll empower individuals to overcome physical limitations, perform everyday activities with confidence and enjoy a better quality of life.

As a Home Health Speech Language Pathologist , you will:

  • Evaluate, direct and provide speech/language pathology service to patients in the home or facility
  • Participate in the development and periodic review of the Plan of Treatment and Plan of Care.
  • Use professional skills and judgment in assessing and treating disorders of speech, voice, language, hearing and swallowing to prevent, identify, evaluate and minimize the effects of such disorders and conditions.
  • Administer and interpret diagnostic tests and applications of therapeutic treatments including audio logic screening.
  • Observe, record and report changes in the patient’s condition and response to treatment to supervisor and/or the physician.
  • Provide instruction and training to patients in use of alternative communication systems when appropriate.
  • Provide counsel and instruction to patients, families and healthcare staff.
  • Maintain and submit documentation as required by the Company and/or facility. Prepare and submit written reports of evaluations, visits, summaries, care plans, care coordination activities, and progress reports as required by Company policy.
  • Participate in care coordination activities and discharge planning.
  • Maintain the highest standards of professional conduct concerning information that is confidential in nature. Share information only when the recipient’s right to access is clearly established and the sharing of such information is clearly in the best interests of the patient.

Required Qualifications:
  • Meet the education and experience requirements for Certification of Clinical Competence in Speech Language Pathology or Audiology granted by ASHA
  • Minimum six months experience as a speech therapist / speech language pathologist
  • Current and unrestricted license
  • Current CPR certification
  • A valid driver’s license, auto insurance, and reliable transportation are required.
Preferred Qualifications:
  • Home Health experience

Pay Per Visit/Unit Rate
$54.00 - $76.00

TB Statement:

This role is considered patient facing and is part of Humana’s Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.

Driving Statement:

This role is part of Humana’s driver safety program and therefore requires an individual to have a valid state driver’s license and are expected to maintain personal vehicle liability insurance. Individual must carry vehicle insurance in accordance with their residing state minimum required limits, or $25,000 bodily injury per person/$25,000 bodily injury per event /$10,000 for property damage or whichever is higher.

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.

$85,400 - $117,500 per year
