Job summary

PeaceHealth seeks a Speech Pathologist Home Health in Florence, Oregon for a full-time position. The role involves evaluating patients, creating treatment plans, and delivering speech-language therapy in the patient’s home environment. You will work as part of a multidisciplinary team and supervise clinical fellows and assistants.

Requires Oregon licensure, CCC-SLP, and 1 year of Home Health experience. A flexible schedule and potential relocation assistance are noted.

Qualifications

• Master's or Doctorate in Speech-Language Pathology (ASHA-certified) is required.
• 1 year of experience in Home Health is required.
• Licensed Speech Pathologist - Oregon.
• CCC-SLP certification required.
• BLS/CPR certification required (ARC or AHA).
• Valid driver’s license and auto insurance proof.

Responsibilities

• Evaluate patients and establish plan-of-care and treatment goals.
• Provide speech-language pathology treatment within plan-of-care.
• Document accurately and timely per licensure requirements.
• Collaborate with healthcare professionals to ensure coordinated care.
• Mentor and supervise students and new staff as appropriate.
• Contribute to development of new services and quality improvement.
