Job summary

Classic Home Care, a nurse-owned skilled home health agency serving Southeast Michigan since 2003, with an office in Northville, is Joint Commission-accredited and Medicare-certified, ensuring high-quality patient care. We seek a motivated Speech Language Pathologist to join our contingent home health team and collaborate with a dedicated RN-led group.

The SLP will provide services per the physician’s plan, travel to patient homes, and contribute to care planning and patient education.

Qualifications

• Active license to practice speech therapy in Michigan.
• Reliable transportation to travel to patient homes.
• Valid driver’s license and auto insurance in your name.
• Home health care experience preferred.

Responsibilities

• Provides speech pathology or audiology services per physician plan of treatment.
• Assists in assessing patient functioning and developing/ revising care plans.
• Records and reports patient responses to treatment to physicians/care managers.
• Advises patient and family on speech pathology and prevention strategies.
• Recommends hearing tests and checks hearing aids when necessary.

We are seeking a motivated and compassionate Speech Language Pathologist (SLP) to join our team as a home health / home care SLP! This contingent position is perfect for a quick learner who thrives in a collaborative, team-oriented environment.

Pay:

• Visit Rates with Mileage Reimbursement: $63.00 - $100.00 per visit


• Discharge Without a Visit: $5.00 - $10.00 per non-visit discharge


• Transfer Without a Visit: $10.00 per non-visit transfer


• Orientation and In Service Hours: $30.00 per hour


• Non taxed mileage reimbursement is paid at $0.50 per mile after 30 miles per day


• Visit Rates without Mileage Reimbursement: $68.00 - $105.00 per visit

Job Description:

• Provides services in speech pathology or audiology in accordance with the physician’s plan of treatment. Accepts only patients referred by the patient care manager or designee. Demonstrates competency in the skills required by the agency on at least a biannual basis.


• Assists the physician in assessing the patient’s level of functioning and the development (and revision, as necessary) of the plan of care by applying appropriate tests for speech, hearing and language disorders. Sets appropriate goals for the patient.


• Records and reports the patient’s reaction to treatment and any changes in the patient’s condition to the physician and/or patient care manager and case manager.


• Instructs and advises the patient, family and other health team personnel in the phases of speech pathology in which they may assist the patient. The goal is to teach and promote disease prevention for eventual patient & caregiver independence, utilizing agency materials as appropriate.


• Recommends hearing tests by audiologist when necessary. Checks hearing aides.

Requirements:

• Active Michigan Speech Therapy license


• Reliable transportation


• Valid driver’s license and auto insurance in your name as a driver


• Home health care experience preferred

Degree of Travel:

• Home visits and office meetings as indicated. Must participate in necessary in-service programs. Must have reliable transportation and agency-required auto insurance.
