Optimal Care - Evergreen is seeking a Home Health Speech-Language Pathologist to evaluate and treat speech, language, cognitive, and swallowing disorders in patients at their homes and in the community. Travel within the service area is daily, with an office location in Carmel, IN.
You will develop individualized treatment plans, collaborate with physicians and the care team, and document care to meet regulatory standards. The role operates Monday through Friday, 8 a.m.

About the Role
Help patients regain their ability to communicate, swallow safely, and improve their quality of life from the comfort of home. As a Home Health Speech-Language Pathologist (SLP), you'll evaluate and treat speech, language, cognitive, communication, and swallowing disorders through individualized therapy plans designed to help patients achieve greater independence and reach their functional goals. Working under physician orders and the established plan of care, you'll deliver evidence-based therapy that supports meaningful recovery and enhances everyday living.
This role is ideal for a compassionate, patient-focused Speech-Language Pathologist who enjoys building relationships and making a lasting impact. You'll perform comprehensive evaluations, develop and implement individualized treatment plans, collaborate with physicians and the interdisciplinary care team, educate patients and caregivers, and maintain accurate clinical documentation that meets regulatory and reimbursement requirements. Through your clinical expertise and commitment to exceptional care, you'll help patients overcome communication and swallowing challenges while supporting Optimal Care's mission of delivering high-quality, patient-centered healthcare.
