Job summary

AdvisaCare is seeking Speech-Language Pathologists to join our growing home health therapy team in Michigan communities including St. Clair Shores, Eastpointe, Grosse Pointe, Monroe, and Belleville.

Provide one-on-one evaluations and individualized treatment plans to help patients improve communication, swallowing, cognition, and function in the home setting. You’ll work with physicians and the care team, document thoroughly, maintain CMS and payer requirements, and educate patients and families

Qualifications

• Degree in Speech-Language Pathology required.
• Current Michigan SLP license in good standing.
• CCC-SLP preferred, where applicable.
• Home health experience preferred.
• Ability to work independently within an interdisciplinary team.

Responsibilities

• Provide one-on-one speech-language therapy in patients' homes.
• Conduct evaluations of communication, cognition, voice, and swallowing.
• Develop and execute plans of care with measurable goals.
• Deliver evidence-based interventions for speech, language, cognition, and swallowing.
• Educate patients and families on strategies and home exercise programs.
• Document care timely per CMS, payer, and agency requirements.
• Collaborate with physicians and care team to achieve patient outcomes.

What You'll Do

As a member of our home health therapy team, you'll provide personalized, one-on-one care to help patients improve communication, cognition, swallowing, safety, and overall quality of life in the comfort of their homes.

SLPs will:

• Complete comprehensive speech-language evaluations to assess communication, cognition, voice, and swallowing needs
• Develop individualized plans of care and measurable treatment goals
• Provide evidence-based interventions for speech, language, cognitive-communication, and swallowing disorders
• Assess and treat conditions including dysphagia, aphasia, dysarthria, cognitive impairment, and other communication disorders
• Reassess patient progress and modify treatment plans as clinically appropriate
• Provide skilled speech therapy services in accordance with physician orders and CMS Conditions of Participation (CoPs)
• Educate patients and caregivers on communication strategies, swallowing precautions, aspiration prevention, cognitive techniques, adaptive tools, and individualized Home Exercise Programs (HEPs)
• Maintain timely, accurate documentation in accordance with CMS, payer, and agency requirements
• Collaborate with physicians, caregivers, nursing, therapy, and the interdisciplinary team to support successful patient outcomes

Requirements

Our Ideal HHC Guru

You may be our next Home Health Magic Maker if you have:

• Degree from an accredited Speech-Language Pathology program
• Current Michigan Speech-Language Pathologist license in good standing
• Certificate of Clinical Competence (CCC-SLP) preferred, as applicable
• Home health experience preferred
• Strong clinical judgment, critical-thinking, and problem-solving skills
• Excellent communication, interpersonal, and patient education skills
• Ability to work independently while remaining connected to the interdisciplinary care team
• Reliable transportation, valid driver's license, and current auto insurance
• Familiarity with Medicare/CMS documentation preferred
