Job summary

Interim HealthCare of Northern Indiana seeks a Home Health Speech-Language Pathologist (SLP) to deliver personalized, home-based therapy in Fort Wayne, IN and surrounding areas. This PRN role offers autonomy, flexible assignments, and the opportunity to impact patients’ communication, swallowing, and cognitive function in a familiar home setting.

Qualifications

• Master's or Doctoral degree in Speech-Language Pathology and Indiana license active.
• CPR certification.
• Minimum of 1 year of speech therapy experience, ideally in home health.
• Knowledge of state and federal home health regulations.
• Good clinical judgement, critical thinking, strong communicator, empathy and compassion.

Responsibilities

• Provide home-based speech therapy to patients with communication, swallowing or cognitive difficulties.
• Collaborate with a home health team (RN, LPN, CNA, HHA, PT, OT) on patient goals.
• Observe and evaluate patient communication, swallowing and information processing abilities.
• Conduct patient assessments, document progress and ensure goals are met.
• Assist with mouth, breathing and swallowing exercises, fluency, articulation and reading.
• Educate patient and family on plan of care, goals, safety and self-care.
