Job summary

BAYADA Home Health Care in Sinking Spring seeks an experienced Speech Language Pathologist (SLP) for a per diem in-home role serving adult and geriatric clients in the local area.

You will work within a multidisciplinary team delivering rehabilitative care, performing thorough evaluations, and developing and adjusting treatment plans. Documentation is on a state-of-the-art tablet; licensure in Pennsylvania and CCC-SLP are preferred, with certification eligibility or progress acceptable.

Qualifications

• Current PA license as a Speech Language Pathologist.
• CCC-SLP or equivalent, or pursuing certification with eligibility to take the exam.
• Experience with EMR systems and in-home care is preferred.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform evaluations to diagnose and treat speech, language, communication, or swallowing issues.
• Develop and revise treatment plans based on evaluations and physician orders.
• Collaborate with multidisciplinary care teams to implement care plans.
• Educate patients and families on rehabilitative care and home safety.
• Document observations and interventions in an EMR on a tablet.
