Job summary

Klarus Home Health in Albuquerque, NM is seeking a compassionate Speech-Language Pathologist to join our team and help patients with speech, language, swallowing, and cognitive disorders receive high-quality home health care.

The role includes planning and evaluating care plans, performing OASIS assessments, coordinating with physicians and caregivers, and delivering therapy with evidence-based techniques in patients' homes, while complying with infection control and continuing education.

Qualifications

• Master’s or Doctoral Degree in Speech-Language Pathology from an accredited college or university.
• Current State License in Speech-Language Pathology or meets 42 CFR §484.115(n); current Driver’s License.
• One year of experience in an acute care, rehabilitation setting, or home health preferred.
• Therapy skills in line with generally accepted standards of practice; good interpersonal skills; proof of current CPR.
• Reliable transportation and valid auto liability insurance.

Responsibilities

• Perform ongoing clinical assessments, including OASIS assessments, to identify patient needs and level of functioning.
• Develop and evaluate the plan of care in partnership with the patient, their representative, and caregivers.
• Prepare and complete clinical notes, phone orders, and other clinical record documentation promptly.
• Analyze patient needs and provide appropriate care as ordered by the physician, revising the plan of care as needed.
• Provide therapeutic treatments for speech, language, cognitive, and swallowing disorders using appropriate tests and measurements.
