Job summary

Graham Healthcare Group is seeking a Home Health Speech Language Pathologist to serve patients in Erie, Pennsylvania. The role offers a competitive salary of $80,000 to $100,000 per year, with a 40-hour work week and an 8-hour shift, working within AHN Healthcare@Home team in a home-based setting.

You will collaborate with clinicians to manage and deliver patient care plans, document care, and travel to patient locations as needed.

Qualifications

• Master’s degree in Speech-Language Pathology and licensure.
• ASHA-certified or state-licensed SLP.
• Minimum of one year healthcare experience.
• Valid driver’s license with ability to drive to patient locations.

Responsibilities

• Deliver and manage patient care plans in home health.
• Collaborate with clinicians to coordinate care.
• Document progress and complete charting timely in EMR.

We are looking for compassionate speech language pathologists with

• Master’s degree from a speech and hearing program certified by the American Speech and Hearing Association to practice in the state in which you work.
• License and certification as a Speech Therapist, issued by the state in which you will work.
• Minimum of one year healthcare experience
• Current driver’s license and ability to spend ~20% of your day driving to/from patient locations
• A commitment to consistently meet critical deadlines for charting
• The skills needed to self-manage your time and schedule
• Demonstrated experience with tablets, mobile phones and EMR software
