Job summary

Penn Medicine in Pennsylvania is seeking a licensed social worker to support patients and families by providing individualized care planning, assessment, and discharge planning in collaboration with the Interdisciplinary Team.

You will assist patients in medical home care services, guide transitions to hospice when appropriate, support advance directives, and help families cope with grief and loss with empathy and professionalism.

Qualifications

• Master of Science in Social Work is required.
• LCSW or LSW licensure is required.
• Experience with medical home care services and interdisciplinary teams is preferred.
• Experience guiding hospice transitions and advance directives is a plus.

Responsibilities

• Communicates with patients and family members in a friendly, caring and informative manner.
• Collaborates with the Interdisciplinary Team to develop and evaluate plans of care.
• Maintains confidentiality and privacy per policy.
• Supports transition planning, including hospice and care transitions.
• Identifies biopsychosocial risks and connects patients to needed resources.
