Iowa Home Care is seeking a dependable and compassionate Home Health Shift Nurse to provide skilled nursing care to patients in their homes around Ottumwa. This role focuses on one patient and family at a time, helping them stay safe and comfortable at home.

Qualifications

• Graduate of an accredited school of nursing (RN)
• Current RN licensure and CPR certification
• Excellent observation, verbal and written communication skills, problem-solving skills; nursing skills per competency checklist

Responsibilities

• Provide skilled nursing services in the patient’s home during scheduled shifts
• Follow the established plan of care and physician orders
• Monitor the patient’s condition and recognize changes that require intervention
• Administer medications and treatments as ordered
• Document vital signs, intake/output, and other clinical information
• Assist with respiratory, nutrition, mobility, and other skilled care needs
• Maintain safety, infection prevention, and emergency interventions
• Communicate changes in patient condition to the care team promptly
• Support patient comfort, dignity, and independence
• Educate and support family members and caregivers
• Build trusting relationships with patients and families while maintaining professional boundaries
• Collaborate with the broader healthcare team and follow policies and regulations
