Job summary

CenterWell Home Health in the United States is seeking a dedicated Home Health Registered Nurse to provide one-on-one care in patients' homes, under the Clinical Manager's supervision. You will develop and adjust care plans, administer medications, and collaborate with physicians and clinicians.

You will educate patients and families, monitor progress, document care, and oversee Home Health Aides. The role emphasizes safe patient transport, staying compliant with TB and driver safety programs.

Qualifications

• Med surg, ICU, ER, or acute experience.
• Current and unrestricted RN licensure.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.
• Valid driver's license and auto insurance.

Responsibilities

• Provide skilled nursing care at patients' homes, including meds, wound care, and assessments.
• Develop and implement individualized care plans with physicians and professionals.
• Monitor patient conditions and report changes.
• Educate patients and families on disease management and treatment options.
• Maintain accurate care records and coordinate with the team.
• Report progress to the patient’s physician and Clinical Manager.
• Oversee Home Health Aides and ensure documentation is completed.
