Qualificações

• Current, active, and unrestricted RN licensure in state of practice.
• Nursing degree from an accredited college or university, or graduate of RN program.

Responsabilidades

• Provides competent nursing care and interventions to home health patients according to state guidelines.
• Supervises care provided by HHAs and LPNs on the care team.
• Collaborates with interdisciplinary team to develop and manage the plan of care.
• Conducts field visits to evaluate start of care and monitor outcomes.
• Completes required documentation (OASIS, SOC/Admissions, Recertifications, ABNs, etc.) timely.
• Maintains patient health records and adheres to safety regulations.
