Assured and Associates is seeking an RN for home health visits to assess and manage patient care in the community. You will visit patients, monitor conditions, take vital signs, and document progress for physicians.

Wound care and medication assistance are part of the daily tasks, with a focus on infection control and confidentiality. High school diploma is required along with an active RN license in the state of practice and CPR/First Aid certification.

Formación

• Must hold an active RN license in the state of practice.
• Current CPR/First Aid certification holder.
• TB test results may be required by policy.
• High School Diploma or equivalent is required.

Responsabilidades

• Provide follow-up skilled nursing visits for home health patients.
• Report patient care/condition/progress to patient's physician on a continuous basis.
• Prepare visit/shift reports and updates/summarizes patient records.
• Assist with prescribed medications for patients.
• Take vital signs as prescribed.
• May record and restock medication inventories.
• Ensure infection control practices are maintained.
• Report hazardous conditions and equipment to supervisor.
• Keep supervisor informed of supply needs.
• Maintain confidentiality of patients' personal care information.
• Act professionally with visitors, staff, and residents.
• Attendance on the job as scheduled.
• Wound care.
