Job summary

CarePartners Health Services in Asheville, NC seeks a Registered Nurse to provide coordinated skilled nursing care to home health patients. You will collaborate with the interdisciplinary team to meet patient goals and ensure high-quality care under the supervision of the Clinical Manager.

Responsibilities include completing OASIS, documenting communications, and coordinating with physicians. You will supervise home health aides and LVNs/LPNs, participate in on-call rotations, and uphold agency

Qualifications

• Basic Cardiac Life Support must be obtained within 30 days of employment start date.
• RN license is required to practice as a registered nurse.
• Valid driver's license is required for home health visits.
• Associate Degree or Bachelor's Degree in Nursing or related field preferred.
• Minimum of 1 year of experience in home health or similar setting.

Responsibilities

• Assess home care patients identifying physical, psychosocial and environmental needs.
• Complete OASIS and visit paperwork per agency policy and document coordination with physician.
• Communicate significant findings to Clinical Manager and physician; document interventions.
• Supervise and direct home health aides and LPNs/LVNs to ensure quality care.
• Participate in on-call rotation and emergency call as per policy.
