Job summary
Visiting Nurse Association of Florida (VNA) is a trusted nonprofit home health provider with 50 years of service. The RN – Home Health is based in Palm Beach County, FL and offers flexible PRN schedules within a mission to help patients heal and remain independent at home.
You will perform skilled nursing visits, complete OASIS assessments, develop and manage care plans, educate patients and caregivers, coordinate with physicians and the inter-disciplinary team, and monitor progress.
Qualifications
- Active Florida RN license required.
- Minimum one year of nursing experience.
- Home health experience preferred.
- OASIS experience preferred.
- Strong assessment and critical thinking skills.
- Ability to work independently.
Responsibilities
- Perform skilled nursing visits in patient homes
- Complete OASIS assessments and documentation
- Develop and manage individualized care plans
- Educate patients and caregivers on disease management and safety
- Coordinate care with physicians and interdisciplinary team members
- Monitor patient progress and communicate changes in condition
