Job summary

• Current and unrestricted RN licensure in state of practice.
• Current Driver's License and access to reliable transportation.

Responsibilities

• Initiates, develops, implements, and revises the plan of care with the physician and other providers.
• Supervises home health aides and licensed staff, providing instruction and task assignment.
• Completes comprehensive assessments (OASIS) and documents visits per policy.
• Communicates with physicians, patients, families, and care teams to ensure quality care.
