Job summary

Millennium Home Care is seeking a licensed Physical Therapist to join our in-home care team in Florida on a PRN basis. You will perform patient evaluations, develop plans of care and deliver manual therapy, therapeutic exercises, and gait training in patients' homes.

Qualifications

• Current and valid Physical Therapist license in Florida.
• Minimum of 1 year of clinical experience as a Physical Therapist, preferably in home health.
• Reliable transportation to visit patients at home.

Responsibilities

• Assess patients and develop individualized treatment plans.
• Implement manual therapy, therapeutic exercises, and gait training.
• Provide functional training and home exercise education.
• Educate patients and families on body mechanics and injury prevention.
• Collaborate with physicians and other clinicians to optimize care.

Responsibilities:

• Patient Assessment: Conduct comprehensive evaluations of patients' physical abilities, limitations, and functional needs to accurately diagnose impairments and develop individualized treatment plans.
• Therapeutic Interventions: Implement a variety of therapeutic techniques, including manual therapy, therapeutic exercises, and gait training to improve patients' mobility, strength, balance, and pain management.
• Functional Training: Assist patients in developing compensatory strategies, adaptive techniques, and assistive device training to enhance their independence in activities of daily living.
• Patient Education: Educate patients and their families on proper body mechanics, home exercise programs, and injury prevention strategies to promote long‑term health and well‑being.
• Collaboration: Work collaboratively with physicians, nurses, occupational therapists, and other healthcare professionals to develop comprehensive care plans and ensure optimal patient outcomes.
• Documentation: Maintain accurate and timely documentation of patient evaluations, treatment plans, progress notes, and discharge summaries.
• Compliance: Adhere to all relevant regulatory standards and guidelines, including HIPAA and Medicare regulations.

Requirements:

• Licensure: Current and valid Physical Therapist license in the state of Florida.
• Experience: Minimum of 1 year of clinical experience as a Physical Therapist, preferably in a home health or a related setting.
• Skills: Strong clinical skills, excellent communication and interpersonal abilities, and a patient‑centered approach to care.
• Professionalism: Strong work ethic, reliability, and a commitment to providing high‑quality care.
