Job summary

Human Touch Home Health Care in Washington, DC is seeking a registered nurse to plan, organize and direct home care services, with emphasis on community health education and outreach within patients' homes.

You will complete initial assessments, regularly re-evaluate nursing needs, initiate and revise the plan of care, administer medications, counsel patients and families, and coordinate care as Case Manager. Pay is per visit, typically one hour, with rates of $65.00–$85.00 per visit.

Qualifications

• Graduate of an accredited school of nursing.
• 1–2 years recent acute care experience.
• Current RN license.
• Bachelor's degree preferred.

Responsibilities

• Completes an initial patient and family assessment to determine home care needs.
• Regularly re-evaluates patient nursing needs and updates care plans.
• Initiates and revises the plan of care as patient status changes.
• Administers medications and treatments as prescribed.
• Counsels patient and family and provides care instructions.
• Acts as Case Manager and coordinates patient care on caseload.
