Job summary

Human Touch Home Health Care in Colorado Springs is seeking a registered nurse to plan, organize and direct home care services, with emphasis on community health education and experience. This role focuses on delivering in-home nursing care to meet patient needs and family education.

The RN provides initial assessments, develops and revises care plans, administers medications per physician orders, and coordinates with the physician and caregivers to ensure quality home health services.

Qualifications

• Graduate of an accredited school of nursing.
• 1-2 years of recent acute care experience in an institutional setting.
• Current Colorado licensure and CPR certification.
• Bachelor's degree with 1 year home health care experience preferred.
• Supervising home health aides not required.
• Strong observation, communication and problem-solving skills.

Responsibilities

• Completes an initial assessment of patient and family to determine home care needs.
• Regularly re-evaluates patient nursing needs and updates the care plan.
• Initiates plan of care and revises as patient status changes.
• Administers medications and treatments as prescribed.
• Coordinates with physician and community resources to implement the plan of care.
• Provides caregiver instruction and counseling to meet patient needs.
• Maintains timely clinical notes and communicates changes to the physician.
• On-call participation and ensuring necessary equipment and services are available.
