Human Touch Home Care in Colorado is seeking a dedicated Registered Nurse to join our home health team. This role focuses on delivering patient-centered care and coordinating plans with a supportive team in a growing home care setting.

Salaries are paid per visit, ranging from $60 to $90, with full-time opportunities, mileage reimbursement, and benefits for eligible staff. State licensure, CPR, and a BSN are preferred, with 1 year of home health experience preferred.

Qualifications

• Graduate of accredited nursing program.
• Current licensure in state and CPR certification.
• Bachelor's degree with one year home health care experience preferred.
• Management experience not required but supervising home health aides expected.

Responsibilities

• Completes initial patient and family assessment to determine home care needs.
• Regularly reevaluates patient nursing needs.
• Initiates and revises the plan of care as needed.
• Administers medications and treatments as prescribed.
• Provides direct patient care per State Nurse Practice Act.
• Counsels patient and family and provides health care instructions.
• Coordinates care as Case Manager and communicates with physicians.
• Assists with discharge planning and coordinates community resources.
• Participates in on-call duties and ensures equipment and services are available.
