Job summary

Akron Children's Hospital in Ohio is seeking a Home Health Skilled Nursing Registered Nurse (RN) for a part-time schedule (0800-1530 or 0900-1630) in the Mahoning Valley. You will deliver comprehensive, patient-centered care in the home setting and collaborate with physicians, therapists, caregivers, and community resources.

The role requires an Ohio RN license, BLS, and at least 1 year of hospital or home care nursing experience; BSN preferred or to be obtained within five years of hire.

Qualifications

• BSN preferred or must be obtained within five years of hire.
• Licensed and registered as an Ohio RN.
• BLS Certification from the American Heart Association required.
• Minimum one year of in-hospital or home care nursing experience.

Responsibilities

• Provide skilled nursing care that meets professional, regulatory, and organizational standards.
• Complete comprehensive patient assessments and ongoing reassessments to guide care planning.
• Develop, implement, evaluate, and revise individualized care plans with the medical team.
• Carry out physician orders and coordinate care with therapy services and community providers.
• Utilize nursing process and clinical judgment to make independent decisions.
• Educate patients and families on disease management, medications, and health promotion.
• Maintain accurate clinical documentation and communicate status changes to the care team.
• Participate in the on-call rotation and QAPI activities.
