Job summary

Enhabit Home Health & Hospice is seeking a licensed healthcare professional to provide intermittent skilled care in patients’ homes across Colorado. You will teach patients, families, and care team members while following physician orders and the care plan.

Qualifications include graduation from an approved clinical education program, active state licensure, and at least one year of licensed professional experience. CPR certification and a valid driver’s license are required.

Qualifications

• Must be a graduate of an approved school of clinical education.
• Must be licensed in the state where they currently practice.
• Must have at least one year experience as a licensed professional.

Responsibilities

• Administer skilled care to patients requiring intermittent professional services.
• Teach the patient, family, and other members of the health care team.
• Perform services in accordance with the physician’s orders and the established plan of care (POC).
