Responsibilities

• Oversee patient care planning, case management, and clinical outcomes.
• Ensure compliance with home health regulations and accreditation standards.
• Serve as a liaison with hospitals, physician practices, and referral partners.

• Maintain accurate documentation and utilize EMR systems effectively.
