Horizon Health in Paris, IL is seeking a Registered Nurse to provide skilled nursing care in patients' homes under the Home Care department. The RN performs prescribed treatments, educates families, and coordinates with other services to support safe, independent living.

The role requires IL licensure, CPR, and at least 1 year of RN experience, preferably acute or LTC. You must have reliable transportation and strong communication, leadership, and documentation skills.

Qualificações

• Registered Nurse with current license in Illinois.
• Minimum 1 year RN experience; 2 years acute/LTC preferred.
• Reliable transportation with Illinois driver's license and proof of auto insurance.
• Willing to participate in quality improvement processes.
• Graduated from an accredited school of Nursing.
• Current CPR certification or within 30 days of hire.
• Ability to communicate well verbally and in writing.
• Knowledge of nursing process and use of the same.
• Ability to perform in a fast-paced environment under stress.

Responsabilidades

• Provide skilled nursing care and prescribed treatments in home care.
• Participates in the on-call system as directed for skilled nursing availability.
• Teaches and supervises family, nursing aides, or others providing care.
• Coordinates with all services in the home.
• Plan with family and physician within available resources.
• Administer medications and supervise administration as prescribed.
