Zusammenfassung

Almost Family, part of LHC Group and aligned with Optum, is seeking a Registered Nurse to provide and coordinate in-home nursing care across Wisconsin. You will assess patients, develop plans of care, and ensure continuity of care in collaboration with physicians, families, and the care team.

The role requires RN licensure in the state, a valid driver’s license, and excellent communication and organizational skills.

Qualifikationen

• Current and unrestricted RN licensure in state of practice.
• Current driver's license, vehicle insurance and access to dependable vehicle or transportation.
• Strong communication and organizational skills.
• Ability to work independently and adapt to in-home care settings.

Aufgaben

• Provide high-quality clinical nursing services within scope of practice and infection control standards.
• Coordinate care with the patient’s care team from admission to discharge.
• Complete nursing assessments per program requirements and payer needs.
• Ensure plan of care frequency and duration meet patient needs and adjust with physician approval.
