Job summary

Virtua Health is seeking a Home Health Registered Nurse to provide skilled nursing care to adult patients in their homes within designated territories. A minimum of one year in medical/surgical care is preferred, and prior home health experience is a plus. Clinical education and patient teaching are core responsibilities.

Qualifications

• Must have valid NJ RN License. Licenses must be presented per NJ Licensing Board.
• Demonstrates knowledge of nursing skills, hospital practices, procedures and standards.
• Presents ongoing education to stay current in nursing practice.
• Strong communication skills are essential.
• Must coordinate efforts of a care team.

Responsibilities

• Assess the patient to develop, implement and evaluate a plan of care.
• Administer and document therapeutic interventions.
• Effectively manage rapidly changing situations.
• Participate in clinical decision-making.
• Educate patient and family on disease process and care needs.
• Delegate and monitor care rendered by the nursing team.
• Demonstrate standard precautions and patient safety.
• Support orientation and education of the healthcare team.
• Participate in performance improvement activities.
• Maintain professional competencies.
