Job summary

UnitedHealth Group seeks an experienced Registered Nurse for Home Health in New Jersey. You will deliver nursing care at patients’ homes as prescribed, coordinate the plan of care with the physician and health team, and ensure compliance with laws and agency policies.

Responsibilities include overseeing home health aides, performing OASIS assessments, documenting visits, and participating in on-call rotations.

Qualifications

• Current and unrestricted RN licensure in state of practice.
• Current driver's license, vehicle insurance, and access to dependable transportation.
• Strong communication and organizational skills.
• Ability to work independently.

Responsibilities

• Initiates, develops, implements, and revises the plan of care in collaboration with the physician and other health care professionals.
• Supervises care provided by home health aides and licensed practical/vocational nurses, and assigns tasks according to regulations.
• Provides comprehensive nursing care in the patient’s home and coordinates with physicians and family.
