Job summary

Enhabit Home Health & Hospice in Grapevine, TX is seeking a licensed healthcare professional to provide intermittent skilled care in patients' homes. You will educate families, follow physician orders, and implement the plan of care to support patient well-being and comfort.

Must be a graduate of an approved clinical program, licensed in Texas, with at least one year of licensed experience and basic technology skills.

Qualifications

• Must be a graduate of an approved school of clinical education.
• Must be licensed in the state where they currently practice.
• Must have at least one year experience as a licensed professional.
• Must have basic demonstrated technology skills, including operation of a mobile device.

Responsibilities

• Administer skilled care to patients requiring intermittent professional services.
• Teach the patient, family, and other members of the health care team.
• Perform services in accordance with the physician’s orders and the established plan of care (POC).
