Job summary

Mercy Home Health, part of Trinity Health At Home, is seeking a Home Care Registered Nurse (RN) to deliver high-quality, patient-centered care in patients’ homes. The role is based out of Nazareth Hospital and will support Northeast Philadelphia, with a focus on advanced nursing skills and compassionate, whole-person care.

You will work in a home health setting, performing phlebotomy and wound care, and collaborating with a multidisciplinary team to help patients heal and regain independence.

Qualifications

• Graduate of an accredited nursing program.
• Active RN license in the State of Ohio.
• At least one year of professional nursing experience, preferably in Home Health.
• Phlebotomy and wound care experience required.
