Job summary

System Support seeks an RN Case Manager to provide home care, following physician-ordered care plans and documenting care delivered. Day, evening, and some weekend shifts are available with flexible scheduling and no on-call requirements.

Registered Nurse licensed in Ohio with CPR certification is required; 6 months acute care experience preferred. Full-time or part-time roles, mileage reimbursement provided.

Qualifications

• Current Ohio Registered Nurse license in good standing.
• CPR Certification (American Heart Association)
• 6 month clinical experience in acute care setting preferred

Responsibilities

• Deliver home care services to patients within the RN scope of practice.
• Facilitate implementation of the care plan as outlined by physician.
• Ensure timely and accurate documentation of care delivered.
• Regularly report to the nursing supervisor on patient status and progress.
