Alumus in Arizona is seeking a dedicated Registered Nurse to provide home-based nursing care, applying evidence-based practices and patient-centered coordination for optimal outcomes. You will perform initial evaluations, update care plans, and deliver services requiring specialized nursing skills while documenting progress in a timely manner.

Qualifications

• Must hold a current, valid RN license in the appropriate state.
• CPR certification required.
• Minimum of one year of verifiable RN experience.
• Recent LTC/SNF/Rehab experience strongly preferred.

Responsibilities

• Performs nursing care using established nursing processes to support home patients.
• Makes initial evaluation visits and regular reevaluations of patient needs.
• Initiates the plan of care and necessary revisions.
• Provides services requiring specialized nursing skills and documents findings promptly.
