Trilogy Home Healthcare, a Humana company, is hiring a dedicated Registered Nurse (RN) to join our home health team in Delray Beach, FL. This full-time role offers PRN options and a chance to provide compassionate care to patients in their homes.

You will collaborate with physicians and the clinical team, manage care plans, and advocate for patients to promote health and independence day by day.

Qualifications

• Graduate of an accredited school of nursing.
• Active Florida RN license.
• Valid Florida driver's license, active vehicle insurance, and clear MVR.
• Minimum 1 year of Acute Care or Med/Surg experience; Home Health experience is a plus.
• Strong initiative and independent judgement.

Responsabilités

• Deliver compassionate, high-quality care to home health patients.
• Collaborate with doctors, Clinical Team Managers, and interdisciplinary teams.
• Be an advocate for patients and help manage care plans.
