Job summary

Responsibilities
- Perform skilled nursing visits in patient homes.
- Complete OASIS assessments and documentation.
- Develop and manage individualized care plans.
- Educate patients and caregivers on disease management and safety.
- Coordinate care with physicians and interdisciplinary team members.
- Monitor patient progress and communicate changes in condition.
