Qualifikationen

Aufgaben

• Observe and monitor patient conditions and perform assessments to develop an individualized care plan.
• Administer medication as prescribed and prioritize visits for high-risk patients to reduce re-hospitalizations.
• Manage multi-disciplinary care with appropriate admissions, transfers, and discharges.
• Educate patients and families using teach-back methods for understanding.
• Complete all clinical documentation following agency protocol and Medicare/Federal guidelines.
• Collaborate with the interdisciplinary team and physicians to ensure optimal care.
• Understand and follow agency policies and communicate schedule/availability changes.
• Other duties as assigned.
