Graham Healthcare Group is seeking a compassionate RN case manager to join our extraordinary care team in Northbrook. In this role, you will lead and collaborate to ensure the management and delivery of patient care plans.

Qualifications

• One year of RN experience and the clinical competence to deliver quality patient care.
• Current driver's license and ability to spend ~20% of your day driving to/from patient locations.
• Demonstrated experience with tablets, mobile phones and EMR software.

Responsibilities

• Lead and collaborate to provide management and delivery of patient care plans.
• Support patients in Northbrook and surrounding areas.
• Meet critical deadlines for charting.
