Job summary

Qualifications

• Current and unrestricted RN licensure in state of practice.
• Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
• Preferred: CPR certification or ability to obtain within 90 days.
• Preferred: 1+ year(s) of home health experience.

Responsibilities

• Provide and direct nursing care to patients in their homes as prescribed by the physician.
• Coordinate total plan of care with health care professionals and patient/family.
• Perform supervisory visits and guide home health aides and LPNs/LVNs.
• Complete assessments (OASIS) and document visits per policy.
