Job summary

Qualifications

• Associate's Degree in Nursing or Diploma Certificate in Nursing.
• At least six months RN experience or at least 1 year of LPN experience, preferably in home care or hospice.
• PC Experience/Skills.

Responsibilities

• Provide professional nursing care to patients in their home with assessment, planning, and evaluation.
• Follow physician orders and ensure documentation reflects care progress.
• Coordinate care with the interdisciplinary team and document actions accurately.
• Identify payor requirements and maintain appropriate visit frequencies and authorizations.
• Update care plans within two weeks of status changes and document Plan of Care.
• Educate patients and families with effective teaching strategies and resources.
