Job summary

Mercy Home Health is seeking a skilled RN for community-based patient care in home settings. You will deliver high-quality care in patients' homes, coordinating with clinicians and family caregivers while documenting to Mercy standards.

The role emphasizes autonomy, diverse patient needs, and a flexible schedule, including 8:00 AM–4:30 PM weekdays with rotating on-call weekends and holidays. Mileage reimbursement is provided.

Qualifications

• RN license in Missouri required.
• Graduation from an accredited nursing program required.
• BLS Certification required.
• Valid driver’s license and proof of auto insurance.
• Prefer one year of Home Health or related experience.

Responsibilities

• Provide day-to-day patient care in the home setting.
• Case management and total care of multiple patients with different levels of acuity.
• Manage admissions to home care services, post-hospitalization assessments, recertifications and discharges.
• Document care using the Epic system.
