Job summary

Pathways Healthcare is seeking an in-home registered nurse to assess and manage patient care in Brookline, MA and surrounding areas. The role involves developing and updating care plans with patient and family input, delivering medications, and coordinating with physicians.

You will work a Monday–Friday day schedule with a strong focus on high-quality, compassionate care. A nursing degree and current BLS CPR certification are required, along with EMR proficiency and a valid driver's license.

Qualifications

• 2+ years’ nursing experience, preferably in geriatrics is desirable.
• Degree in Nursing from an accredited institution.
• Proficient with electronic medical records.
• Excellent bedside manner and communication skills.
• Working knowledge of health care laws and regulations.
• Maintain current BLS CPR certification.
• Possess a valid driver’s license and own a reliable, insured vehicle.

Responsibilities

• Assesses and evaluates patient’s status by writing and initiating plan of care.
• Regularly re-evaluating patient and family/caregiver needs.
• Participating in revising the plan of care as necessary.
• Develops a care plan that establishes goals based on nursing diagnosis and caregiver involvement.
• Initiates preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed.
• Completes, maintains and submits accurate clinical notes and records.
• Communicates with the physician regarding patient needs and changes in condition; obtains orders.
• Provides and maintains a safe environment for the patient.
• Attends interdisciplinary group meetings.
• Participates in on-call duties.
• Assists with equipment and service arrangements.
• Supervises ancillary personnel when required.
• Pursues ongoing professional development and education.
