Job summary

UPMC Home Health is hiring a full-time RN to support patients in Crafton, Carnegie, Greentree and surrounding areas in Western Pennsylvania. We offer a robust career ladder and Total Rewards to support your nursing journey.

Shifts are Monday–Friday daytime with rotations for on-call and weekends; flexible shifts are available, and you will work one-on-one with patients in their homes, with growth opportunities through the nursing career ladder and Total Rewards.

Qualifications

• Active RN license in Pennsylvania or multistate license.
• BLS/CPR certification required.
• 0-2 years nursing experience preferred; acute care, skilled facility, home health, or hospice experience a plus.

Responsibilities

• Establish patients' plans of care with physicians, patients and UM professionals.
• Coordinate case management across a multi-disciplinary team.
• Deliver, initiate and evaluate patient- and family-centered health teaching.
• Support patients in achieving optimal health outcomes in the home setting.
• Complete required documentation per regulatory standards.

Responsibilities

• Your role as the nurse will establish patients’ plans of care in collaboration with physicians, patients, and utilization management (UM) professionals to meet physicians’ orders and patients’ needs.
• RN will coordinate case management activities across a multi-disciplinary health care team to ensure appropriate, quality patient care.
• Initiate, deliver, and evaluate patient- and family-centered health teaching.
• Strive to help patients achieve optimal health outcomes, delivering compassionate care with an emphasis on the patient\'s experience.
• Complete all required documentation including visit assessments, OASIS, 485 and verbal orders accurately and promptly on a laptop or in written documentation while in the patient\'s home and according to all applicable regulatory standards.
• Attend compliance training and adhere to the organization’s standards of conduct, policies, and procedures in addition to all local, state, and federal regulatory guidelines.
• Zero (0) to two (2) years of experience preferred.
• Six (6) months of nursing experience in an Acute Care or Skilled Care Facility or Home Health or Hospice Care preferred.
• BSN preferred.

• Current licensure either in the state where the facility is located or, if the facility is in a state covered by the multistate Nursing Licensure Compact (NLC) agreement, a multistate license issued by a participating NLC state. Hires and current employees working on an out-of-state NLC license who later change their residency to the state where the facility is also located will have 60 days upon changing their residency to apply for licensure within that state.
