Job summary

Lifecare Home Health seeks a skilled Registered Nurse to administer nursing care at patients' homes, coordinate with the interdisciplinary team, families, and referral sources. The RN will perform assessments, develop and revise care plans, and ensure continuity of care.

The role requires current RN licensure and CPR, with preference for BSN. Candidates should be able to travel locally, work flexible hours, and engage in ongoing professional development while ensuring high-quality patient

• Excellent interpersonal communication with patients and families.
• Demonstrates proven decision-making skills.
• Must read, write and comprehend English.

Responsibilities

• Promote /exemplify the Company's mission, vision, and values at all times.
• Complete initial and ongoing assessments to identify client needs and identify care plans; completes assessments including OASIS.
• Regularly re-evaluate client nursing needs and outcomes of care; assist in recovery or maintenance of life functions.
• Develop, initiate, and revise the Plan of Care to ensure quality and continuity of care; initiate preventive and rehabilitative procedures.
• Provide effective communication to clients, families, team members, and other health care professionals; participate in Case Conferences.
• Coordinate services and inform physicians of changes in condition and needs.
• Monitor cases for third-party payer compliance.
• Complete documentation timely to ensure policy compliance; supervise and provide direction to other nursing personnel.
• Perform on-call responsibilities and maintain ongoing professional growth via in-service programs and continuing education.
