Job summary

TexMed Home Health, Inc is seeking a compassionate Registered Nurse to plan, organize, and direct home health services in clients’ homes and communities. You will perform initial and ongoing assessments, develop individualized care plans with the Case Manager, administer medications, and educate patients and families to support independent living.

Qualifications

• Registered nurse with current licensure to practice in the state.
• Graduate of NLN-accredited school of nursing.
• Maintains CPR certification.
• Current valid driver’s license.
• Minimum of two years’ experience, public health/home care preferred.
• Self-directed and able to work with minimal supervision.
• Excellent observation, problem-solving, verbal and written communication skills.
• Able to lift, position, or transfer patients and stand/walk for extended periods.

Responsibilities

• Complete an initial home health assessment to determine care needs.
• Perform initial, comprehensive, and ongoing patient assessments including hospice needs.
• Conduct home visits, assess condition and progress toward goals, provide nursing care and education.
• Develop and revise the plan of care with the Case Manager based on patient needs and goals.
• Administer medications and treatments as prescribed by the physician.
• Counsel patient and family, provide health instructions, and coordinate care with the team.
• Prepare clinical notes and update physicians and the Medical Director as needed.
• Attend on-call duties and participate in quality improvement activities.
