Job summary

ExcelliSearch, Inc. is seeking a Registered Nurse in Brooklyn Park to perform comprehensive nursing assessments and lead personalized care plans. The role involves collaboration with an interdisciplinary team, skilled nursing interventions, and patient-family education.

You will supervise LPNs/LVNs and aides, participate in admissions and on-call duties, and ensure accurate clinical records while delivering compassionate care across all shifts.

Qualifications

• Graduate from an approved school of professional nursing and currently licensed to practice as a registered nurse in the state of operation.
• At least 1 year experience as a RN.
• CPR certification must be valid while employed in a clinical role.
• Must have a valid driver’s license with insured vehicle and be able to travel to all business locations.

Responsibilities

• Perform comprehensive nursing assessments addressing physical, emotional, psychosocial, and spiritual needs.
• Collaborate with the interdisciplinary team to develop, implement, and evaluate personalized care plans.
• Provide skilled nursing interventions including wound care, medication management, IV therapy, and symptom/pain control.
• Deliver education and emotional support to patients and caregivers.
• Supervise LPNs/LVNs and home health aides while coordinating care activities and documentation.
• Respond to urgent needs, participate in admissions/on-call, and adjust plans based on ongoing monitoring.
• Ensure compliant clinical records and high-quality care delivery.
