Responsibilities

• Initiates, develops, implements, and revises the plan of care with physician and health care professionals.
• Supervises home health aides and licensed nurses per state/federal regulations; provides instruction and assigns tasks.
• Provides required supervisory visits.
• Completes comprehensive assessments (OASIS) and medication reconciliation accurately and timely.
• Documents patient visits per policy and payer requirements; coordinates with care team.
