Job summary

Adara Home Health, Inc. is seeking a Clinical Supervisor in Mendota Heights, MN. The role directs clinical activities of clinicians, plans services, and ensures quality care in compliance with professional standards.

Reports to Area Manager and can act as Area Manager in short-term absence. This is a full-time, in-office position with a Monday–Friday schedule and on-call rotation. Responsibilities include directing client care, supervising staff, and maintaining care quality and regulatory

Qualifications

  • Must be a licensed Registered Nurse in Minnesota with no restrictions on the license.
  • Provide proof of negative TB testing (mantoux, TB blood test, or chest x-ray).
  • Must have access to a dependable automobile for travel to multiple company locations.
  • Valid driver’s license and current auto insurance when using a personal vehicle for company business.
  • U.S. citizenship or valid Alien Work Permit.
  • Disclose any conviction/criminal history related to health services; may disqualify.
  • Pass initial and ongoing background checks including MDH and OIG lists.

Responsibilities

  • Lead and mentor clinical staff; ensure services meet professional standards and payer requirements.
  • Oversee daily branch clinical operations for quality and cost-effective care.
  • Plan and direct delivery of client care, assign cases, delegate tasks, schedule visits.
  • Ensure recruitment, onboarding, supervision, evaluation, training, and discharge of staff per policy.
  • Provide clinical oversight, education, orientation, and training for licensed and unlicensed personnel.
  • Review clinical records for state/federal compliance and company policies.

The Clinical Supervisor is responsible for directing the clinical activities of assigned clinicians at a company location; planning clinical services to clients and conferring with employee to ensure services are provided at quality levels consistent with professional standards of practice. The position reports to the Area Manager, and is qualified to act as the Area Manager in a short term absence.

Major Responsibilities


  • Reflects the company's mission and values in developing and maintaining professional working relationship with others.

  • Ensures client care is delivered according to established care guidelines, Plan of Care, physician and non-physician practitioner (NPP) orders, agency policies and procedures and meets insurance and payer requirements.

  • Manages the daily clinical operations of the branch to ensure clients receive quality-assured, cost effective and compliant care as efficiently as possible.

  • Plans, directs and participates in the delivery of client care services, including case assignment, delegation of tasks, case conferencing, ordering medical supplies, and visit scheduling activities.

  • Mentors and supervises staff, holding them accountable for role responsibilities, competencies, individual performance and goals.

  • Ensures recruitment, onboarding, supervision, evaluation, training, assignment and discharge of staff is done according to established company policies and procedures.

  • Provides oversight and guidance on communicable disease management for clients and employees.

  • Monitors and evaluates client care and outcome data and implements processes to continuously improve delivery of quality care to clients.

  • Demonstrates leadership by providing clinical oversight, education, orientation and training to licensed clinicians, office personnel, and unlicensed personnel.

  • Reviews clinical record documentation to ensure compliance with state and federal regulations and company policies and procedures.

  • Communicates and coordinates clinical care needs between the clinicians and the office.

  • Leads multidisciplinary team in regularly scheduled clinical meetings and case conferences.

  • Participates in the after-hours on-call process to ensure client care policies and procedures are followed and staffing issues resolved.

  • Develops and maintains a positive relationship with referral sources including physicians, hospitals, nursing homes, community resources, insurance companies and payers.


PHYSICAL/ENVIRONMENTAL DEMANDS


  • Client care provided in individual client homes and may require transfers and lifting with adaptive equipment. Must be adaptable to a variety of environments and community settings. Must be able to drive or use public transportation in all types of weather.

  • Heavy. Amount of weight moved; 50 to 100 pounds occasionally; 25 to 50 pounds frequently; 10 to 20 pounds constantly. Light. Amount of weight moved; up to 20 pounds occasionally.

  • Office work requires frequent sitting, standing, bending, and stretching.


Qualifications


  • Currently licensed as a Registered Nurse (RN) in the State of Minnesota. The license must not have been revoked, suspended and without limitations or restrictions

  • Proof of negative TB testing: mantoux skin test, single TB blood test or chest x-ray.

  • Access to a dependable automobile in order to travel to multiple company business stops a day.

  • A valid driver's license and proof of current car insurance when using a personal vehicle for company business.

  • Have U.S. Citizenship or evidence of valid Alien Work Permit.

  • Disclose any conviction and criminal history records pertaining to any crime related to the provision of health services. A candidate who has been convicted of such crimes will not be hired.

  • Pass initial and ongoing background studies and screenings including but not limited to those of the Minnesota Department of Health and the Federal Office of the Inspector General’s List of Excluded Individuals and Entities.


Skills And Experience


  • Have at least two (2) years’ experience in a health care facility or home care field. Home Health Care preferred.

  • Demonstrates critical thinking skills, the ability to follow standards and procedures, and make appropriate clinical judgments to provide safe and effective client care.

  • Ability to work independently adapt to change, and manage a variety of assignments while establishing priorities and ensuring the necessary duties are completed.

  • Ability to build and maintain professional relationships.

  • Excellent written and oral communication skills; interpersonal communication and teaching skills.

  • Basic computer skills and willing to learn new software programs; experience with Electronic Medical Records and Microsoft Outlook preferred.

  • Current CPR certification highly recommended


Adara Home Health, Inc. is committed to fair and equitable compensation practices. The base salary range for this position in the U.S. is $83,000 - $100,000 per year. A candidate's salary is determined by various factors including, but not limited to, relevant work experience, skills, certifications, job level, supervisory status, and location.
