Job summary

CenterWell Home Health in Indianapolis is seeking a Registered Nurse Clinical Manager to lead a team of clinicians delivering high-quality home-based care. You will guide clinical practice, coordinate patient services, and drive quality improvement while supporting staff to achieve independence and optimal outcomes.

Reporting to the Branch Director, this on-site role involves Medicare/compliance focus, OASIS accuracy, documentation audits, on-call coverage, and ensuring regulatory readiness with

Qualifications

• Graduate of an accredited School of Nursing.
• Current state RN license.
• CPR certification required.
• Minimum 2 years RN home health experience.
• OASIS experience preferred.
• Management and people leadership experience required.

Responsibilities

• Guide, support, and educate clinicians; assist with care planning and clinical decisions.
• Ensure documentation, audits, and billing meet Medicare standards.
• Participate in hiring, training, performance evaluation, coaching, and corrective action.
• Review OASIS, visit notes, and plan of care for accuracy and compliance.
• Collaborate with field clinicians to ensure timely submission and resolve deficiencies.
• Assist with OASIS accuracy and validation; align data entry with care plans.
• Monitor quality indicators and develop performance improvement plans.
• Participate in quality improvement projects and regulatory surveys.
• Track and report quality metrics to leadership.
• Provide staff education on compliance and documentation.
• Stay current on federal/state home health regulations.
• Provide direct patient care on limited basis in exceptional circumstances.
